4.7.4.2 5W questioning technique

In problem solving, it is important to identify the root cause of a problem and to correct it. It is often found that the immediate cause is not the fundamental cause because it has itself been triggered by another cause. It is therefore important not to work on (intermediate) symptoms, but to get to the bottom of the actual causes. In the 5W questioning technique, the respective reason is asked until there is no more answer to this question – one feels reminded of the questioning technique of a small, curious child. Experience shows (although not always) that after asking five times, one will have found the causal reason for one’s problem, hence the name 5W question technique (5 times why?).
